How do I access a
Day Centre services?
The Italian Day Centre cares for home and community care clients. Clients would normally be referred to the
centre by:

Initial assessment is carried out by Day Centre Coordinator.
If services provider by the Day Centre are not suitable, referral would be made to another
Service Provider. If excepted by the Centre then the cost for the service would be
$8.00 covering meal and Day Centre activities. If transport is provided by
Community Transport there is a set minimal cost.
Copy of booklets covering rights and responsibilities are available at the Centre.
If the services are refused, or you are dissatisfied with our service you have the right to make a complaint in accordance with Day
Centre policy and discuss any concerns in the presence of an advocate of your choice with the coordinator.
The objective of the Centre is to help clients to maintain their independence at
home and in the community, thereby preventing or delaying admission to long-term residential care. The
Centre does this by providing culturally appropriate information, activities and meals in a friendly, supportive and
safe environment.
The Outreach worker provides services to clients in their own home. This also includes
assessment or reassessment and provides a care plan for clients and referrals to other appropriate service providers.
Services also include social support, translation of correspondence and assistance with appointments.
Day Centre Volunteers will receive referrals when The Centre is aware of who would like visitation in their home, hospital etc.
This process is provided after consultation with clients.
